We will review your medical history and determine if you meet eligibility per your state regulations to obtain a medical card. NH residents must have their initial appointment with us in person per state regulations. Afterwards, they can recertify via telehealth. MA residents can utilize telehealth or in-person for any of their visits. Please email a copy of your medical diagnoses from your PCP/specialist as well as a copy of the front of your driver's license to support@faroutalternativecare.com. If you do not have medical documents proving your diagnoses you should choose the "requiring diagnosis" option so our provider can have a more in-depth consult evaluating you for a qualifying diagnosis,

We use a variety of approaches in our practice to address your concerns. This may be vitamin and nutrition therapy alone, or in combination with hormonal replacement therapy. Each plan will be individualized based on the patient and their symptoms.
