Initial Certification Consult
For new patients of Far Out Alternative Care. All new patients must have an initial consult with our Nurse Practitioner to determine eligibility.
Diagnosis Consult
For anyone who does not have documentation of a qualifying condition. Use this option to add to your visit if you will need a more thorough evaluation for diagnosis of possible PTSD, anxiety, or depression.
